| 目的 探讨风险分级管理在上海市郊区糖尿病患者中的应用效果。方法 将上海市某郊区120例住院糖尿病患者随机分为对照组和实验组。对两组患者分别进行糖尿病足风险分级。对照组患者进行常规口头教育，实验组患者根据风险等级的不同进行分级管理。结果 干预后，两组患者糖尿病足部护理知识及行为水平均得以提升，但实验组提升更显著，差异有统计学意义（P<0.05）。随访一年后，实验组患者踝肱指数（ABI）明显高于对照组（P<0.001）；对照组2例患者发生糖尿病足，发生率为4%，中高危足患者占26%；实验组无患者发生糖尿病足，中高危足患者占17.9%。结论 糖尿病足风险分级管理，可提高糖尿病患者足部护理知识及行为水平，降低糖尿病足风险等级及糖尿病足溃疡的发生率。 |

| Objective To explore the effect of risk classification management of diabetic foot on diabetic patients in suburb of Shanghai.Methods 120 hospitalized diabetic patients in a suburb of Shanghai were randomly divided into control group and experimental group. The risk factors of diabetic foot were classified in two groups. The patients in the control group were given routine oral education, and the patients in the experimental group were given classification management according to the risk level.Results After intervention, the foot care knowledge and behavior level were improved in both groups, but the improvement was more significant in the experimental group,The differences were statistically significant（P<0.05）.After a year follow-up,ankle brachial index(ABI) of experimental group patients was higher than that of control group patients,and there were 2 patients suffered from diabetic foot in the control group,the incidence rate was 4%.Meanwhile,there were 26% of the control group patients with middle or high diabetic foot risk .While in the experimental group,there was no patients suffered from diabetic foot .And the patients with middle or high diabetic foot risk was 17.9%.Conclusion Classification management according to the diabetic foot risk level can improve the foot care knowledge and behavior level、reduce the risk level of diabetic foot and the incidence of diabetic foot ulcer. |
