探索由二级医院牵头,联合紧密医联体下的社区卫生服务中心,利用“熙心健康APP的“熙心养护”平台,实施“互联网+护理服务”上门服务工作。目前通过700多例上门服务案例,为长期卧床的高龄老人、行动不便等居家患者,提供专业的上门护理服务,发挥了专科护士的积极作用,促进了医联体之间优质资源的共享,“互联网+护理服务”模式在社区得到了推广及应用,周边老百姓反响较好。

英文摘要:

To explore the implementation of "Internet + nursing service" under the leadership of the second level hospital, in conjunction with the community health service center under the close Medical Union, and by using the "Xixin maintenance" platform of "Xixin health app". Through the practice of nearly 600 home-based service cases for more than two years, we have provided professional home-based nursing services for the elderly who have been bedridden for a long time and the home patients with mobility problems, played the positive role of specialized nurses, and promoted the sharing of high-quality resources between medical associations. "Internet + nursing service" mode has been promoted and applied in the community.
