      目的 探讨葡萄糖酸氯已定抗菌透明敷料在血液透析中心静脉临时导管中的应用效果。方法 选取2021年4月~2022年6月在我院肾脏内科通过中心静脉临时导管行血液透析的住院患者共128例,按随机数字表法分为对照组和观察组,每组各64例。观察组在临时血透导管出口处使用葡萄糖酸氯已定抗菌透明敷料,对照组使用医用敷贴。对两组患者导管出口处感染、导管相关性血流感染、局部皮肤过敏情况及导管敷料固定情况进行比较。结果 观察组导管出口处感染和敷料卷边发生率明显少于对照组,两组比较有统计学差异(X2=3.905,X2=7.143;P<0.05);观察组导管相关性血流感染率为1.05‰,低于对照组5.57‰;观察组临时导管移位和局部皮肤过敏发生率低于对照组,两组比较无统计学差异(X2=1.574,X2=0.508;P>0.05)。结论 葡萄糖酸氯已定抗菌透明敷料能帮助护理人员持续观察穿刺点,有效降低导管出口处感染及敷料卷边发生率。

      Objective:To investigate the application effect of chlorhexidine gluconate antibacterial transparent dressing in central venous temporary catheter of hemodialysis. Methods : A total of 128 inpatients who underwent hemodialysis through a central venous temporary catheter in the Department of Nephrology of our hospital from April 2021 to June 2021 were selected and divided into a control group and an observation group according to the random number table method, with 46 cases in each group. The observation group used chlorhexidine gluconate antibacterial transparent dressing at the outlet of the temporary hemodialysis catheter, the control group used medical sterile dressing. The catheter outlet infection, catheter-related bloodstream infection, local skin allergy and catheter dressing fixation were compared between the two groups. Results :The incidences of infection and dressing curling at the catheter outlet in the observation group were significantly lower than those in the control group, and the difference was statistically significant between the two groups(X2=3.905,X2=7.143;P<0.05) ; catheter-related bloodstream infection rate in the observation group 1.05‰ was lower than the control group 5.57‰; the incidence of temporary catheter displacement and local skin allergy in the observation group was lower than that in the control group, and the difference was not statistically significant (X2=1.574, X2=0.508; P>0.05). Conclusion :Chlorhexidine gluconate antibacterial transparent dressing can help the nursing staff to continuously observe the puncture point and effectively prevent infection at the outlet of temporary hemodialysis catheter and the incidence of dressing curling.
