| 目的：调查肿瘤患者放疗期间发热的发生情况并分析原因。方法：2020年2月～2021年1月在本院进行放射治疗住院患者,每天监测体温,统计患者放射治疗期间发热的发生情况及分级,并分析发热的可能原因。结果：共收治患者2374例,对其中102例(4.3%)发热患者进行分析。男性65例(63.7%),女性37例(36.3%)；中位年龄60.5岁(范围34～84岁)；单纯放疗33例(32.4%),放疗联合药物治疗例(67.6%)。低热22例(21.6％),中度发热患者59例(57.8％),高热21例(20.6％)。发热持续中位时间为2.5天(1～14天)。肿瘤性发热48例(47.1％),感染性发热34例(33.3％),中性粒细胞缺乏伴发热15例(14.7%),药物性发热 5例(4.9％),感染性发热中以呼吸道感染最为常见。所有患者均根据病因进行了相关对症处理,无1例患者因发热终止治疗或死亡。结论：发热是肿瘤患者常见的症状,在放射治疗期间,肿瘤患者发热的程度以中度发热为主,最常见的原因为肿瘤热,粒缺发热,及上呼吸道感染发热,及时早期识别发热原因,有助于进一步对症治疗,保证各项治疗的顺利完成具有重要意义。 |

| Department of Radiation Oncology, Renji Hospital, Medical College, Shanghai Jiaotong University, Shanghai 200127, China: Objective: To investigate the occurrence and analyze the etiologies of fever in cancer patients during radiotherapy. Methods: Patients undergoing radiotherapy between February 2020 and January 2021 were included in this study to monitor their body temperatures, record the occurrence of fever in the process of radiotherapy and analyze potential causes. Results: During the study, 2374 patients were admitted by the hospital, and 102 were diagnosed with varying degrees of fever, accounting for 4.30% of all inpatients. Among these patients, 21.6% (22/102) had a mild fever, 57.8% (59/102) presented with moderate fever, and 20.6% (21/102) were diagnosed with a high fever. These fever cases were categorized as noninfectious fever (neoplastic fever), infectious fever and drug-induced fever, representing 47.1% (48/102), 48.0% (49/102) and 4.9% (5/102) of all fever cases, respectively. Of the patients with infectious fever, 14.7% (15/102) were associated with neutropenia and 13.7% (14/102) were caused by upper respiratory tract infection. Conclusion: As a symptom commonly seen in cancer patients, fever occurs during radiotherapy.predominantly at a moderate degree mostly because of cancer, neutropenia or upper respiratory tract infection. Patients can benefit from the early identification and differentiation of the exact etiology , receive symptomatic treatment and achieve desired therapeutic effects. |
