      目的? 探讨两种浓度肝素封管液应用于留置动脉微导管患者的临床效果及对患者凝血功能的影响。方法 拟选取行股动脉化疗栓塞术后留置微导管的100名患者,简单随机分为A组、B组,A组使用12.5 U/ml的肝素钠封管液、B组使用6.25 U/ml的肝素钠封管液进行封管处理。观察两组患者动脉微导管堵管、渗血情况;统计动脉微导管堵管发生率、动脉微导管局部渗血发生率以及术前、拔除微导管后的凝血指标,并对数据进行统计学分析。结果 12.5 U/ml组APTT(活化部分凝血活酶时间)、PT(凝血酶原时间)拔管后时间较术前延长且有统计学意义,6.25 U/ml组APTT、PT术前、拔管后无统计学意义;12.5 U/ml组APTT、PT拔管后时间较6.25 U/ml组延长且有统计学意义。12.5 U/ml发生堵管1例,6.25 U/ml组无堵管情况发生;高浓度组患者渗血发生率为18%,高于低浓度组的16%,两组渗血情况无统计学意义。结论 使用6.25 U/ml浓度的肝素液进行封管,能减少对患者凝血功能影响的同时,不会增加堵管的风险。

      【】 Objective? To investigate the clinical effect of two concentrations of heparin sealing solution in patients with indwelling arterial microcatheter and its influence on coagulation function. Methods 100 patients with microcatheter after femoral artery chemoembolization were simplerandom sampling divided into Group A and Group B. Group A was treated with 12.5 U/ml heparin sodium sealant and Group B with 6.25 U/ml heparin sodium sealant. Observe the arterial microcatheter blockage and local bleeding in the two groups; The incidence of arterial microcatheter blockage, local bleeding of arterial microcatheter and coagulation indexes before operation and after removal of microcatheter were counted, and the data were statistically analyzed.Results? The time of APTT (activated partial thromboplastin time) and PT (prothrombin time) after extubation in 12.5 U/ml group was significantly longer than that before extubation, but there was no significant difference between APTT and PT before and after extubation in 6.25 U/ml group. The time of APTT and PT after extubation in 12.5 U/ml group was longer than that in 6.25 U/ml group. Tube blockage occurred in 1 case at 12.5 U/ml, but no tube blockage occurred in 6.25 U/ml group. The incidence of bleeding in 12.5 U/ml group was 18%,and the incidence of bleeding in6.25 U/ml group was 16%, and there was no statistical significance between the two groups.Conclusion? The use of heparin solution with a concentration of 6.25 U/ml for tube sealing reduces the impact on the patient"s coagulation function while not increasing the risk of tube blockage.
