| 目的 调查本院相关员工、住院患者及其主要照护者对于医院安全文化认知和预防跌倒认知的现状,探索两者之间的相关性,为改善住院患者跌倒预防措施寻找线索,为设计和建设优质的医院安全文化体系提供依据。方法 采用问卷调查的方法,在2022年7月-8月从医方及患方两方面进行预防跌倒知信行问卷、医院安全感知水平的问卷(PMOS)、医院病人安全文化调查问卷(HSOPSC)。对上海市徐汇区中心医院933名员工及711名患者及家属进行调研。医方共发放问卷933份,回收788份,有效问卷764份,患方共发放问卷711份,回收654份,有效问卷632份。结果 医患双方对医院安全文化总体评价为中等(护士、医生及医技、工勤、护工等医方的医院安全文化总分分别为149.01±16.64,143.32±17.14,134.33±10.65,166.64±15.12;患者、主要陪护者等患方的医院安全文化总分分别为111.24±9.75,109.07±11.08)。不同岗位员工对各维度安全文化认知评分的差异具有统计学意义(值172.44,P≤0.05)。患方在“尊严和尊重”、“医患沟通与团队合作”、“组织和护理计划”和 “信息交流”四个维度上,患者的评价均高于主要陪护者的评价,且差异具有统计学意义(值7.7,P≤0.05)。医方对预防跌倒的知信行认知情况总体较好的比例较高,分别为81.1%、66.9%。医患双方在跌倒预防知识方面的得分均为中等以上;信念方面,护理人员中得分较好者比例最高(72.73%),其他人员得分中等者比例较高(65.5%);行为方面,医患双方得分均为较好(84.1%)。跌倒预防知信行水平与其感知的医院安全文化水平具有显著的相关性(Pearson 相关系数0.1-0.56,P≤0.05)。结论 我们要从跌倒预防的流程入手,进一步细化,优化流程,多部门协作,增进沟通,医护,辅助人员、患者、家属共同合作,形成管理模式,最终营造患者安全文化及氛围,保障患者安全。 |

| Objective: To investigate the current status of hospital safety culture awareness and fall prevention awareness among relevant employees, inpatients, and their main caregivers in our hospital, explore the correlation between the two, find clues for improving fall prevention measures for inpatients, and provide a basis for designing and constructing a high-quality hospital safety culture system. Method: A questionnaire survey was conducted From July to August 2022, including a fall prevention knowledge, belief, and behavior questionnaire, a hospital safety perception level questionnaire (PMOS), and a hospital patient safety culture survey questionnaire (HSOPSC) from both medical and patient perspectives. A survey was conducted on 933 employees and 711 patients and their families at Xuhui District Central Hospital in Shanghai. The medical team distributed a total of 933 questionnaires, with 788 responses and 764 valid responses. The patient team distributed a total of 711 questionnaires, with 654 responses and 632 valid responses. The overall evaluation of hospital safety culture by both doctors and patients was average (the total scores of hospital safety culture for nurses, doctors, medical technicians, work attendance, and caregivers were 149.01 ± 16.64143.32 ± 17.14134.33 ± 10.65166.64 ± 15.12, respectively; the total scores of hospital safety culture for patients, main caregivers, and other patients were 111.24 ± 9.75109.07 ± 11.08, respectively). There is a statistically significant difference (value 172.44, P ≤ 0.05) in the perception scores of safety culture among employees in different positions. In terms of "dignity and respect", "doctor-patient communication and teamwork", "organization and nursing plan", and "information exchange", the patient's evaluation was higher than that of the main caregiver, and the difference was statistically significant (value 7.7, P ≤ 0.05). The overall proportion of medical professionals with good knowledge, belief, and practice in preventing falls is 81.1% and 66.9%, respectively. Both doctors and patients scored above average in terms of fall prevention knowledge; In terms of beliefs, the proportion of nursing staff with good scores is the highest (72.73%), while the proportion of other staff with average scores is higher (65.5%); In terms of behavior, both doctors and patients scored well (84.1%). There is a significant correlation between the level of knowledge, belief and practice in fall prevention and the perceived level of hospital safety culture (Pearson correlation coefficient 0.1-0.56, P ≤ 0.05). Conclusion: We need to start with the process of fall prevention, further refine and optimize the process, collaborate with multiple departments, enhance communication, cooperate with medical staff, auxiliary personnel, patients, and family members, form a management model, and ultimately create a patient safety culture and atmosphere to ensure patient safety. |
